Expression of Interest


Please provide details below to gain access to all relevant information regarding participation in the 2026 HWEI Employee survey.

If your organisation operates more than one area, and you feel it would be advantageous for you to be able to separate the responses of employees in these different areas (for instance your organisation operates in both 'health' and 'education') you may request more than one survey link. 

Please ensure that when you fill out the 'organisation' field, you clarify that each link is for a different area of your organisation.

E.g. ACON- Health Services,  ACON - Education Services 

NOTE: Completing this expression of interest is not a commitment to participate in the survey. However, if you decide not to proceed, please let us know.

A separate EOI is required for each LINK you wish to be sent

Communications Person
 Please provide the best email for you/your network or person responsible for the distribution of the survey within your organisation. A group/generic email may be used to have communications go to multiple people, e.g. D&I team, LGBTQ ERG.This email address will receive all updates and correspondence until the close of the survey period (closes February) This person will not receive the results at the end of the survey



Who Can Participate?


Participation in the HWEI Employee Survey is exclusively available to Pride in Inclusion Program members as part of your membership benefits.

To cover the cost of development, running, marking, data analysis, and reporting, Pride in Diversity, and Pride in Health + Wellbeing have created a ‘Digital and Index Participation‘ membership tier, designed for organisations that only wish to participate in the HWEI Submission process, Employee Survey, or both.

If you are not a member, you can continue to express your interest, and documents will be provided once your membership is confirmed. 


Results Person 

Per ACON’s Pride Inclusion Programs' Research Data Security Policy (which will be supplied), participating organisation's will receive their data at summary level if sufficient responses are received. If the organisation receives sufficient responses, they will also be provided de-identified line-by-line data. 

It should be noted that this is confidential employee information; while de-identified, some populations will be small when multiple filters are applied, e.g., location, age, gender, gender experience, etc. Therefore, it is recommended that organisations also ensure their own internal data sharing protocols are applied to maximise anonymity of respondents.

Please provide the name and contact details of your Senior HR person. Survey results will only be sent to this person.

HWEI Survey Agreements. 

By signing and submitting this form you are agreeing to the following conditions: 

  • You have permission from your organisation to request and review the survey information

  • You will obtain authorisation from your organisation prior to distributing the survey unique URL throughout your organisation

  • You will distribute the unique URL assigned to your organisation exactly as it was sent by Pride in Health+Wellbeing; it will not be altered in any way

  • The primary contact email provided will be included in future communications regarding HWEI survey data and updates during the open survey open period

  • The senior HR person identified as the results/secondary contact is aware of the confidential nature of the survey results and will ensure your organisation's internal data sharing protocols are applied to maximise anonymity of respondents

  • If your organisation decides not to participate after reviewing the survey information, you will advise Pride in Health+Wellbeing, and the unique link will be decommissioned

  • Any data received at the finalisation of the survey will be maintained in confidence
